I'm wondering if it's not some sort of automatic string waxer?

Secure a knob of wax in the cup/pocket just left of the spool, direct the string through the hole on the left and then pull the string passing it through the knob of wax as it dispenses it so the wax adheres to the string? Might work.

Failing that it's just a fancy schmancy string dispenser :lmao:

Brick line for marking walls? Put coloured chalk powder in the cup and run the string through it, hold it tight to the wall and snap it to leave a line.

Does the hole go all the way through? if so I think the holes a handle socket and the thing is a "dreel marker" a dreel is plants planted in a line, "Working doon the berry dreel". Gardeners use then all the time to mark straight lines.
Here's another type
