Is the handle hollow all the way through? Could you run thread or string through from the right-hand end to the spikey bit (assuming there's room to get past the screw)?

And, does the smaller spikey end have slots as well?

For a horrible moment I thought it was a trocar and cannula but it isn't. As soon as I looked up what one of those was, the difference was obvious.

It looks like the notches are simply to allow the end to be splayed but maintain a straight line rather than a curl. This means that you couldn't use it to punch through a sheet and then draw something back from the other side. A shot of the tool rotated to show the pair of holes might be useful.
